When you submit the contact form, I collect your name, email, phone number (if provided), area of interest, and message. This information is sent to me via email and used solely to respond to your inquiry. It is not stored in a database or shared with anyone.

This Form Is Not HIPAA-Compliant

The contact form is not a secure or HIPAA-compliant method of communication. Information submitted through the form travels over the general internet.

Please do not include sensitive health information, diagnoses, or treatment details in the contact form. For secure communication, contact the office directly by phone at (602) 642-3255.

Once you become a client, all clinical communication occurs through HIPAA-compliant channels as outlined in your intake paperwork.
